What you can expect from us for home care

Our colleagues can help you in all kinds of situations. We believe it is important that you remain as independent as possible. So what you can do yourself, you do yourself. Do you need help to do it yourself? Or are you really unable to do it yourself? Then we are there for you. For example, we help you with your medication. With washing and dressing. And with your medical aids. Think for example of help with your stoma. Or help with changing the dressing on a wound. 

Remote healthcare

It is not always necessary for us to visit you. With the right tools, you can still do many things yourself. And sometimes we can look along remotely. For example, with video calling. Or with a device that prepares medication for you at the right times. In a personal conversation, we will discuss what is best for you.

The right care

We work together with other healthcare providers. For example, the GP post. And the emergency department of the hospital. In this way, we ensure that you receive the right care as quickly as possible.

Introductory meeting

We'll discuss your specific care needs during an introductory meeting. We'll visit you at home for this. Beforehand, we'll ask you to complete a short questionnaire by email. This allows us to better understand your health and wishes. Based on this meeting, we'll determine the best solution together. If we decide to provide care, we'll create a personalized care plan for you. This plan outlines exactly what we'll do, what you can expect from us, and what we expect from you. We'll also always assess whether Mindful Movement is right for you. This 12-week exercise program supports your balance and fitness. It's a standard part of your care.

Extra care via Hups

Are you already receiving care from us and would you like some extra care? That can be nice if your informal caregiver is sometimes unable to help you. Or if you do not have an informal caregiver. You can arrange this care yourself via Hups. You do not need a referral from a doctor or from us for this. Extra help is sometimes reimbursed by the health insurance. Would you like to know more? Help at night

Would you like someone to help you at night if necessary? If you have an indication, you can receive night care from us. Ask your district nurse if this applies to you.

If you do not have an indication for night care, you can contact Hups for help at night, for example with a sleep service.

Question and answer

How do I request home care from TWB?

If you would like to request care from TWB, you can call our Care Center on Phone: 088 560 2000 . We will ask you some questions to get an idea of ​​your situation. Then we will make an appointment for you with a district nurse. This colleague will visit you at home for a personal introductory meeting to see what exactly you need.

Do I need a referral for TWB care?

No. Everyone can apply for care from us. A district nurse will determine in a personal interview whether you actually need care. We call this an indication. 

Do I have to pay for the care myself?

Do you need short-term care? For example after an operation? Then the health insurer will pay for this from the basic insurance. This means that you do not have to pay anything yourself. It is also not deducted from your own risk. You do need an indication from the district nurse for this. You will receive this in a personal introductory meeting. 

If you need intensive care for a long period of time, you can apply for an indication under the Long-term Care Act. This is also called a Wlz indication. You can apply for this via the CIZ. Do you have a Wlz indication? Then it pays health care office a portion of the costs of care. You will also have to pay something for this yourself. How much that is depends on your income. Sometimes you need aids for our care. Think of a shower chair. You often have to pay for these aids yourself.
