Do you require permanent care? Then we are here for you and your loved ones, so that you can continue living at home for as long as possible. To determine what care you need, it is important to consider what you can still do yourself and what aids we can use. And what your family, an informal caregiver, or others in your life can do for you.
Long-term care in your familiar environment
If you have been given a Wlz indication, the CZ care office is responsible for your care provision. Wlz stands for Long-term Care Act. Your care will continue as usual, but from that moment on it will no longer be paid for by your insurance (Zvw) or the municipality (WMO). With a Wlz indication, you can choose from different types of care. You can read below what you can choose from.
Complete package at home
With the complete package at home (vpt) you will receive care at home like in a nursing home. This allows you to continue living at home for as long as possible. With the vpt we arrange all the care you need together with you to be able to continue living at home safely and responsibly. We offer for this:
- personal care
- nursing care
- household support and laundry
- individual or group guidance
- informal care support
- meals
- personal alarm
- transportation for medical appointment
Modular package at home
Are you opting for a modular home package (mpt)? Then you will receive your care from various care providers. You choose your own care providers. And you decide which care you receive from which provider. We offer the following services for the mpt:
- personal care
- nursing care
- household support
- individual or group guidance
Personal budget
You can also receive a personal budget (pgb). This is an amount with which you purchase care yourself. You are then responsible for the administration at the care office. For example, you must ensure that the care office pays the bills of the care providers.
You cannot purchase care from the PGB at TWB. You can convert part of your PGB to MPT. TWB then provides the care and arranges the payment directly with the care office.

What is the Long-term Care Act (Wlz)?
The Long-term Care Act (Wlz) contains all the rules regarding care for people who need a lot of care for a long time. People fall under the Wlz if someone needs to be nearby 24 hours a day to provide care. And sometimes even if someone needs to supervise 24 hours a day. The Wlz mainly applies to vulnerable elderly people, people with a physical disability or people with a mental disorder.
The care office pays for care covered by the Wlz. This is then no longer handled through the health insurer (Zvw) or the municipality under the Social Support Act (Wmo). For our region, the CZ care office manages this.
What is a Wlz indication?
With a Wlz indication, TWB can arrange the right care together with you, so that you can continue living at home for as long as possible. TWB can also support you if you wish to move to a residential care facility. With a Wlz indication, you can be placed on a waiting list for a residential care facility.
How do I apply for long-term care?
To receive care covered by the Wlz, you need a Wlz indication. You can apply for this yourself via the CIZ website.
We can help you with this. Often you will already be receiving care from us at the time that you need long-term care. At that time we will talk to you to see what exactly you need and how we can best arrange this together.
You can also ask a client support worker if they can help you with the application. To do so, contact your municipality's Wmo desk.
Do I have to pay for long-term care myself?
If you have a Wlz indication, the care office pays the majority of the costs. In addition, you pay an income-dependent personal contribution yourself. The amount depends on your income. You can easily calculate the additional payment required on the CAK website .
